Err.. Yes, you did. You just weren't paying attention.

Any death underwater can be traced back to the diver. CCR's (and rebreathers in general) just give you a whole bunch of extra contributing factors. It's your choice: spend a lot of your time monitoring the unit, or risk dying. SCR and OC require successively less monitoring and have more obvious failure modes.
